Click on "Durak's Journal" inside "Forgotten Hold" major landmark, you'll find a sarcophagus with the journal sitting next to it.

Click on "Durak's Tome" on the floor on the lowest level of "Gnol Barak" major landmark, between a pillar and a cannon.
