Organ Grinder is a Tome of Knowledge Bestiary entry for Plaguebearer.

Kill 'Grotesque Plaguebearers' in the Chaos Wastes around 48k 53k until one drops a Pocket Item 'Pestilent Organ'. Equip it for the unlock.

Once the unlock triggered, you can claim the jewel buying it from the correct merchant in Library (Order) / Lyceum (Destruction).
