Lair of Gorthlak is home to a rank 40 Undead Orc named Gorthlak., and is located in The Blighted Isle at 35.6k 18k. It is found northwest of the High Elf Chapter 2, west across the lake from the Order Public Quest Thanalorn Forest.

Approaching the lair will trigger an Achievements unlock. (Not implemented yet)

Directions

To reach the lair, you must follow the pass around 40k 24k. Head north, then swim west through the lake until you reach a beach with crystals. These crystals are parts of Tome of Knowledge unlocks for The Gateway to Ultimate Knowledge. Jump across the stones to the left and enter Gorthlak's lair.

Tip: As Destruction player, to reach order side of the map, I advice you to head 34k 28k, then jump down. You will have to walk on the western side of the ground to avoid 55 Champions of High Elf Chapter, then head to 40k 24k.

Mechanic to make spawn the boss (on Official)

Not implemented yet

Before you right click the 'Tainted Crystal', the cave would be a normal one, with some low level lizards inside. When you activate the 'Tainted Crystal', it transforms into the Lair and you'll find the boss inside.
