First, you have to take the quest "Marked" in Norsca at the Chaos Chapter 2 from Surtsen the Deceiver.
Step by Step Guide
Step 1
Go west to Charred Lands and click on a Corspe Cart to loot item 'Crawling Doll'. Examine the item in your quest inventory to turn in the quest.
Step 2
Return to Surtsen the Deceiver at Chaos Chapter 2.
Step 3
Travel south into Nordland, and speak with Brakr Thornedblade at Chaos Chapter 3.
Step 4
Run southeast and kill 8 Wilds Huntsmen at the PQ 'The Wilds of War'.
Return to Brakr Thornedblade to turn in your quest.
Step 5
Travel south just outside Smugglers' Tunnel and speak with Konrad Schulman.
Step 6
Enter the PQ 'Salzenmund' and find Priest Adelmar at the top of some stairs at 48.5k/58.4k. Talk to him.
Step 7
Go northwest and kill Jakob Ebbe at 40.7k/48k to loot the item 'Garret's Shipment of Pipe Weed'.
Turn in your quest to Priest Adelmar.
Step 8
Talk to Rat Catcher Thomas into Salzenmund PQ at 52.6k/54.7k.
Return to Priest Adelmar.
Step 9
Speak with Emile Ahlstrom at 56.5k/58.3k.
Step 10
Return to Priest Adelmar and speak to him.
Step 11
Find and kill Temple Moneyhandler at 50k/58.5k to loot the item 'Temple Accounting Records'. Then use this item in your quest inventory.
Turn in the quest to Priest Adelmar.
Step 12
Kill Steward Lord Geinhardt at Salzenmund Keep (62k/45.6k) and take his head.
Turn in this last step to Svartr Oleifson at Chaos Chapter 4 and claim your reward.
