This is a Bestiary Tome Unlock for Juggernaut of Khorne.
Destruction & Order
Click on a "Pile of Rubble" at 40k, 15k in the Chaos Wastes for this unlock. This is at the northwest edge of the complex of ruined walls in the area; a vent of shifting, waving aurora is nearby. There is an "Unbound Mastodon" (a Juggernaut of Khorne) just on the other side of a ruined wall from the "Pile of Rubble."

You examine the rubble and discover a mostly decomposed boy beneath the rocks. From your best guess, the body is from an adventurer that sought out the nearby Juggernaut and discovered what happened when he got between him and a hard place.
