Kill "Stonegullet Trolls" in "The Despair Pits" cave at 23k 28k, or just northeast outside, in Black Crag, until one drops a Pocket Item "Troll Flesh". Equip it for the unlock.

Tips

In the same cave "The Despair Pits", you will find a pack of stones "Drul's Lunch" for the unlock Indigestion.

Outside northeast of the cave, you will find "Rockmaw" for the unlock Rock in the Maw of Darkness.
