Brutal Nose Ring is a Tome of Knowledge Bestiary entry for Giant.
Instructions: Order & Destruction
Kill "Vragi the Brute" (Level 39 Chaos Troll) in Kadrin Valley at 33.5k 28.1k, at the top of the cliff edge, to collect a Pocket Item "Giant Nosering". Equip it for the unlock.
